Better Quality with Integrated Supports 

At Values Into Action, we support people to live the life they choose. We also keep learning and planning ahead as people’s lives and the service system change. 

Our 2026–2028 Quality Management Plan, or QMP, guides this work. Its focus is Participant Outcomes and Satisfaction. We want people to have the support, information, and resources they need to work toward their goals.

Integrated supports are the different kinds of support that can work together in a person’s life.

  • Paid services and supports 

When supports work together, people have more choices and more ways to reach their goals. 

What Do We Want People to Experience? 

Through this plan, we want people to be able to say:

What Will We Be Working On? 

Over the next two years, we will focus on: 

  • Helping staff learn more about integrated supports. 
  • Using Charting the LifeCourse tools in planning. 
  • Using health data to drive wellness, and inclusion. 
  • Planning and using creative ideas as needs and services change. 
  • Asking people, families, and staff for feedback. 
  • Reviewing data and making improvements when needed. 

How Will We Know If We Are Making Progress? 

We will learn from: 

  • Surveys and feedback 
  • Progress reports and record reviews 
  • Health and wellness information 
  • Examples of successful supports 
  • Team planning and follow-through activities 

We will use what we learn to strengthen supports and improve outcomes.
